After you enroll, we’ll ask you to provide or update your personal information and preferences, which includes your health information, lodging and meal preferences for your program.

On the Participant Information Form, please indicate to us the name of your roommate/traveling companion. Also let us know if you are traveling with other individuals and wish to be assigned a room near them. Photo ID Three (3) additional forms needed: Road Scholar forms needed: 1. Pre-program Questionnaire. Please fill out the questionnaire prior to the program. The link can be found in the Recommended Websites section of your pre-program information packet. 2. Parental Consent Form: A Parental Consent Form is required for minors and should be returned at least 5 weeks before the program start. This form must be signed by ALL custodial parent(s) / legal guardian(s) of the minor. Completed forms can be emailed to programadmin@roadscholar.org or mailed to Road Scholar, PO Box 56033, Boston, MA 02205. 3. Santa Barbara Adventure Co. Acknowledgement of Risk Multisport and Santa Barbara Adventure Co. Waiver Forms for beach play and snorkeling on Santa Cruz Island. Each participant is required to fill out the waivers. Please follow the links listed in the Recommended Websites section of your pre-program information packet.

United States citizens don’t need a visa for any of the countries you’ll be visiting. Non-U.S. citizens should refer to their country's requirements to ensure they have the necessary documentation for travel.

A signed Parental Consent form is required for this program. The form must be signed by both parent(s)/legal guardian(s) of the child participating in the program and returned to the program provider prior to the start of the program. You will receive the form as part of your program information packet. You will also be able to access the form through your online account.
